Protected Health Information (PHI), about you, is maintained as a written and/or electronic record of your contacts or visits for healthcare services with Richard M. Butlig, MD, Inc.  Specifically, PHI is information about you, including demographic information (i.e., name, address, phone, etc.), that may identify you and relates to your past, present or future physical or mental health condition and related healthcare services.


Richard M. Butlig, MD, Inc. is required to follow specific rules on maintaining the confidentiality of your PHI, using your information, and disclosing or sharing this information with other healthcare professionals involved in your Care and treatment. This Notice describes your rights to access and control your PHI. It also describes how we follow applicable rules and use and disclose your PHI to provide your treatment, obtain payment for services you receive, manage our healthcare operations and for other purposes that are permitted or required by law.


Calls or Text During Office Hours

Patients and caregivers are encouraged to use Patient Ally's portal to communicate securely with our office. We also welcome your call with any questions. The office staff is trained to answer most non-clinical questions, including billing inquiries. Staff members are not able to give medical advice over the phone. If you have a medical-related inquiry, we ask that you leave a message with one of our staff members or message us through Patient Ally portal. Your primary care physician, the doctor in the office at the time, or our medical assistant will return your call or message. 

When leaving messages during office hours, please keep in mind that our priority is with the patients that are in the office. Therefore, your call will be returned once a medical caregiver finishes seeing our patients in the office.

If you are having a medical emergency, CALL 911 or go to your nearest emergency department.  

For urgent matters, please find an URGENT CARE near you.
For NON-EMERGENCY, if you becomes acutely ill after office hours, we recommend referring to the Symptom Checker below from Mayo Clinic or WebMD website for instructions on management.

​Missed or Cancelled Appointments

​​You may be charged for any appointments missed or cancelled without 24 hours notice.

Forms Completion

Completing paperwork/forms for school, Family Medical Leave Act (FMLA) claims, IHSS, long-term care, life insurance, disability claims, unemployment claims, etc goes beyond routine medical care provided to you. Therefore, they cannot be billed to your insurance company.

Forms take a considerable amount of the staff and clinicians’ time to review and complete.

Due to an increased amount of requests from patients to complete paperwork/forms, the following will be implemented.
A. FEE: minimum $25.00 PER form (ADDITIONAL $5.00 per page for pages#3 and more that require ANY section of that page being completed). **certain fees are waived**

B. PAYMENT: Payment is DUE prior to any forms being completed.

C. MAILING/EMAILING/FAXING: We will no longer be able to mail/email/fax completed forms to anyone other than the patient/family. It should be the patient’s responsibility to ensure the proper parties receive the information.

D. TIME FRAME: We will strive to get forms completed in a timely manner. Most of the clinicians work outside of our clinic so it may take longer for them to receive and complete the forms.
Generally, paperwork/forms will be completed between 5-10 business days.

E. Office Visit, Work, and School Note can be provided at patient’s request with no charge if requested on the day of service (these notes usually have verbiage confirming you were seen in our clinic on a specific date).

Richard M. Butlig, MD, Inc. accept most PPO insurance plans in addition to Medicare, Blue Cross, Blue Shield, CIGNA, Aetna.

It's that time again, FLU SHOTS are in!
Flu shot, also known as Influenza vaccines, are vaccines that protect against influenza. A new version of the vaccine is developed twice a year as the influenza virus rapidly changes. While their effectiveness varies from year to year, most provide modest to high protection against influenza. They decrease the number of missed days of work by a half day on average.
