Thank you for taking the time to read our policies, and please talk to any of us if you have questions.

Calls or Text During Office Hours

Patients and caregivers are encouraged to use Patient Ally's portal to communicate securely with our office. We also welcome your call with any questions. The office staff is trained to answer most non-clinical questions, including billing inquiries. Staff members are not able to give medical advice over the phone. If you have a medical-related inquiry, we ask that you leave a message with one of our staff members or message us through Patient Ally portal. Your primary care physician, the doctor in the office at the time, or our medical assistant will return your call or message. 

When leaving messages during office hours, please keep in mind that our priority is with the patients that are in the office. Therefore, your call will be returned once a medical caregiver finishes seeing our patients in the office.

​For NON-EMERGENCY, if you becomes acutely ill after office hours, we recommend referring to the Symptom Checker below from Mayo Clinic or WebMD website for instructions on management or go to Urgent Care near you.

​Missed or Cancelled Appointments

​​You may be charged for any appointments missed or cancelled without 24 hours notice.

Workers Comp, Physician's Report (RCFE) and Other Forms

You may mail, fax or drop-off any forms that you need filled out for work. There is a $25 charge for forms filled out by the doctor or nurse. 

Please send your forms as early as possible as we get many requests prior to the deadline. If your form does not require a doctor's signature, you may request additional information from our office via Patient Ally portal, and fill in the forms yourself.

If your school form does not require a doctor's signature, you may request vaccine information from our office via Patient Ally portal, and fill in the forms yourself. Any school, sports or camp forms brought in at the time of the Well Child Check or Annual Physical Exam will be filled out at no charge.

Major Insurance Providers Accepted

Dr. Richard M. Butlig accept most PPO insurance plans in addition to Medicare, Blue Cross, Blue Shield, CIGNA, Aetna.

It's that time again, FLU SHOTS are in!
Flu shot, also known as Influenza vaccines, are vaccines that protect against influenza. A new version of the vaccine is developed twice a year as the influenza virus rapidly changes. While their effectiveness varies from year to year, most provide modest to high protection against influenza. They decrease the number of missed days of work by a half day on average.
